A lesson from Management of Pacemakers and ICDs in the Perioperative Setting

Postoperative Management of Pacemakers and ICDs: Reprogramming and Interrogation

After surgery, it is critical to ensure that any temporary changes made to pacemakers or ICDs during the procedure are properly addressed:

  • Device Reprogramming: If the device was reprogrammed preoperatively (e.g., set to an asynchronous mode or had ICD tachytherapies turned off), it is essential to have the device reprogrammed back to its original settings postoperatively. This reprogramming should be performed by the electrophysiology team as soon as possible after the surgery to restore the device's intended function.
  • Monitoring: Until the device is rechecked and reprogrammed, the patient should be continuously monitored on telemetry. This is particularly important if the device was altered during surgery, as there may be a risk of arrhythmias or inappropriate pacing. Continuous monitoring ensures that any issues can be promptly identified and managed.
