A lesson from Preoperative Evaluations: Ensuring Patient Safety and Optimizing Surgical Outcomes

Goals of the Preoperative Evaluation for Anesthesia Providers

The goals of the Preoperative Evaluation (POE) encompass several key objectives across different stakeholders in the surgical process. For anesthesia providers, the primary aim is to gain a thorough understanding of a patient's medical history and current medications, ensuring any undiagnosed or poorly managed conditions, particularly those affecting the cardiovascular and respiratory systems, are identified and assessed for their impact on anesthetic care. This step includes evaluating whether risks such as cardiac and pulmonary issues, medication reactions, bleeding or clotting disorders, diabetes, kidney disease, and other organ-related conditions can be mitigated. Anesthesia providers also seek to consult with other healthcare professionals for additional evaluations, plan the appropriate anesthesia, obtain informed consent, and document the anesthesia strategy and airway assessment.

For surgeons and hospitals, the goals include minimizing the cost of perioperative care, shortening hospital stays, reducing the incidence of cancelled or delayed surgeries, optimizing resource use, improving patient outcomes, and enhancing patient satisfaction.

For the patient, the focus will be on education to reduce risk factors (such as smoking cessation, screening for sleep apnea, and correct perioperative handling of medications such as beta-blockers), providing information about the recommended anesthetic and pain management plans, what to expect during and after surgery, reducing anxiety through detailed explanations and emotional support, and advising on medication management pre-surgery.

Goals of the preoperative evaluation for the anesthesia provider, the surgeon and hospital, and the patient

Overall, the collective aim of the POE is to decrease surgical and anesthetic morbidity and mortality by ensuring patients are fit for the procedure and identifying those at high risk of perioperative complications. This comprehensive approach facilitates a safer, more efficient surgical experience for all involved.
